A MESSAGE TO PATIENTS

Re: My Concierge Medical Practice

To Whom It May Concern,

As I complete my 21st year in my Internal Medicine practice, I reflect upon another year of personal change and adjustment to challenges impacting medicine in recent years. It has been a continuous balancing act between taking care of a large number of patients and finding time to give each one the attention they deserve while finding time for my family as they transition to different stages of life. Therefore, after much planning, I have transitioned my practice to a Concierge Model on March 1, 2019.

I spent much of the last year studying for my 3rd Internal Medicine Board exam (taken every 10 years), and I continue to find practicing medicine a very rewarding experience and enjoy treating my patients based on the most current recommendations. I am grateful to have a career in which I get the opportunity to help patients every day and am fortunate that my practice has thrived despite all the changes to medicine in the last 20 years. These modifications have been vast and range from handwritten charts being converted to computerized medical records, hospitalists taking over inpatient medicine, and insurance companies trying to dictate what can and can’t be ordered for patients. These developments have made it increasingly difficult to serve my large number of patients in the effective manner in which I aspire without sacrificing irreplaceable time with family and friends.

In order to continue to provide the necessary attention and focus to patients, I have made a positive transition to a Concierge model of medicine. This yearly fee that is assessed allows me to focus my time on a much smaller number of patients in the office and in the hospital, with more flexibility and availability. This model of practice also allows for consultations over the phone and video conferencing during and after office hours. The enclosed documents provide further details and will hopefully answer many of your questions. Of course, I am happy to personally address any questions you may have regarding my new practice.

I will only be able to take a limited number of patients in order to make this model successful for both myself and for patients, so please notify my assistant Rachael at 312 922-1290 if you are interested and ready for this type of medical care.  After this limited number is met, I will then formulate a waiting list to work with as openings develop.

Finally, I know some of you may not need this type of medical attention and are looking to find a new physician. I would refer you to any of my colleagues in this group and our newest physician, Dr. Sean Connolly. Regardless of your choice, I want all of you to make sure that you have a physician that you trust to focus on your medical needs and help you stay in the best health possible.

Best wishes to a healthy future,
Shirley Lee TerMolen, MD, SC
